If another household member has listed your children's information, please leave blank so as not to duplicate the information. In order for this anonymous questionnaire to be statistically significant, we ask you to please fill out only one form per individual.
This questionnaire is one of the many methods the Search Committee will use in order to effectively develop the church profile. The development of this profile is essential and a prerequisite for reviewing pastoral candidates.
